Community Support Specialist (Northshore PSH)
Job Description
The Permanent Supportive Housing (PSH) program links adults ages 18 and up to affordable rental housing with voluntary, flexible, and individualized behavioral health services to people with severe and complex disabilities, enabling them to live successfully in the community. Community Support Specialist provide CPST (Community Psychiatric Supports and Treatment) and PSR (Psychosocial Rehabilitation) services in home and community based settings.
Minimum Requirements
Bachelor or Master’s Degree in Sociology, Psychology, Social Work or Counseling
Be over 18 years of age
Possess a valid driver's license

Knowledge of agency and community resources and referral processes.
Job Duties/Skills Required
Provides case management and supportive services (social skills, life skills, acquisition of benefits, community resource utilization, job search and placement, coping skills, money management) for clients in home and community based settings.
Incorporates skill building and supportive interventions to restore and rehabilitate social and interpersonal skills and daily living skills.
Completes all required documentation and forms for clients on caseload.
Ascertains resources outside of Start when necessary to meet client’s specific needs.
