Program Director of 5-6 Evidence Based Programs (EBP). Providing supervision and management of EBP's supervisors, referrals, production, goal attainment, hiring, reporting and program growth/development.

Healthy Start Case Manager

Seeking Case manager who provides high quality care to case manage/coordinate women, infants, and fathers to reduce mortality rates in women and infants in our community. Serves as Liaison between medical providers and clients, to help foster effective communication and appropriate plan of care. Is responsible for managing patient workflow for maximum efficiency to ensure productivity is met.

Currently hiring for St John Parish, St James Parish, Assumption parish, and St Charles Parish. Main office located in Houma, must be willing to come 1-2 times a month and meet virtual weekly.

The housing specialist will assist ACT clients in obtaining affordable housing in conjunction with client financial means and/or available grants. The housing specialist will also work with other ACT Team members and specialist to meet additional client needs as directed by ACT Team Lead.

Licensed Mental Health Professional (ACT)

ACT Team LMHP will provide individual and group counseling sessions in the field to individuals with mental health symptoms and needs of high acuity. The LMHP will also assist in conducting comprehensive assessments of new and established clients when needed. The LMHP will work in conjunction with the rest of the team to provide any additional services deemed necessary by the team lead.

Substance Use Specialist (ACT)

Substance Use Specialist works along side a multidisciplinary team, providing community based services to high risk individuals. Team member participates in staff meetings, runs groups, provides direct substance use support/services, assists with treatment planning, and is recovery focused.

Housing Specialist (ACT)

Housing specialist works alongside a multidisciplinary team, going out into the community to provide ACT services. HS assists high risk individuals with housing needs, supports, paperwork, budgeting, treatment planning goals, and is cross-trained.

PSS works in the field, along side a multidisciplinary team providing intensive services to persons with severe and persistent behavioral health disorders. PSS completes treatment plans, hurricane plans, crisis plans, and other ACT specific documents. Models recovery-oriented thinking and behavior.

Housing Advocate 1 (Part-time)

Monitoring Clients, checking residents in and out of the Womenspace shelter, promoting empowerment, encouraging and advocating for the resident, teaching housing skills, hygiene skills, and assisting with community resources.

Licensed Mental Health Professional (ACT Team)

We are seeking a LCSW/LPC to provide individual counseling services to ACT Team clients. May require assessment or reassessment of new and active clients.
