Job Description

Do you have a passion for working with parents and caregivers who are trying to build a stronger, more stable family life? As a Case Manager in the My Community Cares program, you will meet with parents and caregivers to connect them to meaningful supports and resources, develop customized support plans, and work on skills intended to move them from crisis to stability.

Minimum Requirements

  • High School Diploma: At least 5 years’ experience in social services

  • Bachelors level: Social Work, Psychology, Sociology, etc.   

  • At least 2 years’ experience in social services

Core Competencies

  • Effectively demonstrate the understanding and practice of FRC and MCC programs. 

  • Communicate respectfully and effectively and establish professional relationships. 

  • Promotes recovery, independence, and personal choice for participants.  

  • Assists participants in identifying strengths, needs, and realistic goals to develop a service plan. 

  • Accurately identifies risk and responds appropriately.  

  • Demonstrates advanced knowledge of behavioral health symptoms, interventions, and impact. 

  • Maintains professional boundaries and operates within agency /ethical guidelines.  

  • Knowledge of agency and community resources and referral processes.

Job Duties/Skills Required

  • Provide consistent, quality case management and skills based services while maintaining fidelity to MCC programs.  

  • Schedule and maintain a flexible workweek to meet the needs of FRC families, scheduled program meetings and documentation responsibilities. Sometimes requires working nontraditional, flexible hours, including evenings, weekends and holidays.  

  • Ability to work with diverse individuals/families/populations. 

  • Work is performed in family homes, the community and an office environment.  

  • Transport families to scheduled appointments throughout the community in agency vehicle. 

  • Complete documentation in an electronic health record and update program spreadsheets. 

  • Involves exposure to tobacco smoke, domestic animals, household pests, and other conditions common to home environments.
