Community Support Specialist (QIF-TJI)
Job Description
The Community Support Specialist in QIF provides case management and support for individuals recently released from incarceration at Lafourche Correctional Complex, and the Terrebonne Parish Criminal Justice Center.
Minimum Requirements
Bachelor’s degree in Sociology, Psychology, Counseling, or a related field
At least 18 years of age
Possess a valid driver’s license
Peer Support training (preferred)
Personal experience of incarceration, homelessness, and/or SUD/COD (preferred)

Job Duties/Skills Required
Deliver trauma-informed, pre- and post-release case management services for all participants, including conducting assessments, developing individualized case plans, and maintaining accurate records in the Electronic Health Record (EHR) system.
Support participants’ successful transition into the community through consistent engagement, warm handoffs, and appropriate referrals to address individual needs.
Maintain regular contact with each participant throughout the duration of the program to ensure continuity of care and goal progression.
Collaborate with the Project Care Team to discuss case management strategies and coordinate care.
Assist participants in identifying their strengths, needs, and achievable goals through active listening, empathy, and unconditional positive regard.
Facilitate connections to community resources and services, support benefit eligibility and acquisition, and coordinate appointments, reminders, and transportation as needed.
Conduct initial client assessments (triage) for providers by gathering relevant medical, psychological, and social history (including Social Determinants of Health) in a sensitive and efficient manner.
Provide comprehensive case management and supportive services designed to improve participants’ overall well-being and address social determinants of health.
Transport participants to appointments using a company vehicle, when necessary.
Perform other duties as assigned by the Program Director to support program operations and client success.
