Veteran Services Outreach Coordinator (FOX Prevention)
Job Description
The Veterans Services Outreach Coordinator provides services to locate and engage Veteran's in the community who may be at risk or in need of specialize resources and/or services.
Minimum Requirements
Bachelor Degree in social services field
High School degree or GED with at least 2 years experience in social services
Be over 18 years of age
Possess a valid driver’s license

Knowledge of agency and community resources and referral processes.
Develops and maintains cooperative relationships with agencies and stakeholders.
Facilitates referrals to Start Corporation programs.
Good interpersonal and communication skills.
Promptly and accurately completes required documentation, data entry, and reporting.
Job Duties/Skills Required

Provides outreach services to locate and engage persons in the community who may be at risk or in need of specialized resources and/or services.
Interviews clients and/or families and completes assessments and other clinical documentation.
Completes case logs and required documentation on all client services including data entry into required data systems. Ensures that clinical documentation and client files meet all applicable standards for licensing, accreditation, or other regulatory agencies.
Consults with facility personnel and administrative field personnel; collects and analyzes data to identify needs for the assigned special services program within the particular service area.
Effectively utilizes community resources by providing appropriate referrals and linkages.
Maintains a clear driving record in order to be able to drive company vehicles to provide transportation to clients.
Knowledge of all related privacy and confidentiality requirements including HIPAA and laws related to confidentiality regarding substance abuse, HIV status, and treatment.
Develop outreach strategies with community partners to better understand our programs, services, and referral process.
Coordinate with marketing department to strategize outreach campaigns and participation in community events.
Organizes or attends conferences, trade shows, marketing events, fundraisers, health fairs, etc.
