Job Description

We are looking for an Assessment Navigator for our Assertive Community Treatment Program to help connect new referrals who are experiencing severe behavioral health symptoms to ACT services by coordination and facilitation of telehealth assessments. This position is community-based traveling to multiple parishes around South East Louisiana.

Minimum Requirements

  • High School Diploma or Equivalent 

  • Experience with SMI Preferred

  • Be over 18 years of age (required) 

  • Possess a valid Louisiana’s driver’s license or Louisiana State ID (required)

  • Knowledge of agency and community resources and referral processes.

Job Duties/Skills Required

  • Coordinate scheduling and transportation for assessments required for Medicaid services. 

  • Facilitate Telehealth assessments as needed to ensure timely access to services. 

  • Collaborate with ACT Business Development Specialist, ACT Program Director, and clinic personnel to ensure all referrals receive an initial assessment as scheduled.  

  • Work closely with Program Managers, Team Leaders, Community Support Specialists, and clinic staff to ensure all referrals are responded to, scheduled, and complete within 30 days of referral.  

  • Maintain working knowledge of service levels, including but not limited to Mental Health Rehabilitation (MHR) and Assertive Community Treatment (ACT). 

  • Document all assessment coordination activities in the client’s record in AWARDS. 

  • Identify and coordinate external resources when client needs extend beyond internal capabilities. 

  • Consult with facility and field personnel to analyze service needs and support program improvement. 

  • Perform other duties as assigned by the Program Director. 

  • Meet with Program Director and ACT Business Development Specialist regularly to review all referrals’ status of completion.

  • Assess barriers to connection to services and report to ACT Business Development Specialist and Program Director immediately as issues arise.
