Patient Assessment Navigator (NOLA FQHC)
Job Description
Assist with assessment through telehealth; provide transportation to the clinic as needed; work with a multidisciplinary team; and has experience with working with patients for primary and behavioral healthcare needs.
Minimum Requirements
Bachelor degree preferred
Core Competencies
Understanding of primary and behavioral health and terminology.
Knowledge of community resources and referral process.
Job Duties/Skills Required
Participate in the triage of patients that are in need of behavioral and primary care services.
