Job Description

We are hiring for a full-time Community Support Specialist! This role will provide trauma-informed, pre- and post-release case management for all participants including conducting assessments, creating individualized case plans, maintaining case records in the EHR, supporting the transition into the community, providing warm handoffs, and referrals to meet participants’ needs.

Minimum Requirements

  • Bachelor’s degree in sociology, psychology, or counseling. 

  • At least 18 years of age. 

  • Possess a valid driver’s license. 

  • Peer Support training preferred. 

  • Personal experience of incarceration, homelessness, and/or SUD/COD preferred. 

  • Knowledge of community resources and/or willingness to learn about and develop relationships with these resources. 

  • Willingness and eagerness to learn and grow both personally and professionally. 

  • Sincere desire to help others, non- judgmental, and openness to new experiences are key personality trait

  • Promotes recovery, independence, and personal choice for participants. 

  • Demonstrates knowledge of behavioral health symptoms, interventions, and impact. 

  • Maintains professional boundaries and operates within agency / ethical guidelines.

  • Knowledge of agency and community resources and referral processes.

Job Duties/Skills Required

  • Will provide trauma-informed, pre- and post-release case management for all participants including conducting assessments, creating individualized case plans, maintaining case records in the EHR, supporting the transition into the community, providing warm handoffs and referrals to meet participants’ needs. 

  • The CSS will maintain contact with each participant throughout the duration of the program. 

  • Confers on case management as part of Project Care Team. Assists clients in identifying strengths, needs, and realistic goals for service and care planning through active listening, empathy, and positive regard. 

  • Facilitates linkage with community service providers, aid client in benefit eligibility/acquisition, coordinates appointments, reminders, transportation. 

  • Conduct initial assessments of clients/patients (triage) for providers, which entails gathering medical, psychological, and social information (SDOH) in a sensitive and efficient manner. 

  • Provide comprehensive client case management and supportive services to improve social determinants of health (SDOH). 

  • Provide transportation (if needed) in company vehicle to all appointments. 

  • Assist with other duties as requested by Program Director.
