Healthy Start Case Manager
Job Description
Seeking Case manager who provides high quality care to case manage/coordinate women, infants, and fathers to reduce mortality rates in women and infants in our community. Serves as Liaison between medical providers and clients, to help foster effective communication and appropriate plan of care. Is responsible for managing patient workflow for maximum efficiency to ensure productivity is met.
Currently hiring for St John Parish, St James Parish, Assumption parish, and St Charles Parish. Main office located in Houma, must be willing to come 1-2 times a month and meet virtual weekly.
Minimum Requirements
Associate’s or Bachelor’s Degree in social services or related fields (public health, health promotion, psychology, sociology, counseling, etc.)
Ability to obtain a driver’s license and CPR/First Aid certification
Must have a valid driver’s license and reliable transportation
Must meet the minimum required level of auto insurance per carrier guidelines
Previous experience and familiarity with the culture, socioeconomic characteristics, and language of theservice population is required.

Job Duties/Skills Required
Service Planning: Assists clients in identifying strengths, needs, and realistic goals to develop a comprehensive service plan.
Social & Household Skills Training: Effectively teaches social skills and household management skills.
Clinical Care: Provides high-quality care including assessment, triage, treatments, and documentation incompliance with standards of care.
Liaison Role: Serves as a liaison between medical providers and clients to foster effective communicationand appropriate care plans.
Patient Workflow Management: Manages patient workflow efficiently to meet productivity standards.
Care Implementation: Reviews treatment plans with supervising RN, implements processes, and assistswith ongoing patient assessment and reevaluation.
Data Collection & Reporting: Collects clinical data, records, and reports changes in patient conditions,prioritizing data based on patient’s immediate needs.
Comprehensive Care: Provides care based on the whole person, addressing health, nutrition, andclothing needs while recognizing individual treatment needs.
Non-Judgmental Care: Provides care in a non-discriminatory manner that is sensitive to patient andfamily diversity, preserving autonomy, dignity, and rights.
Staff Education: Provides educational in-services focused on universal precautions, reducing infectiousdisease exposure, and proper procedures for handling non-compliant clients, seizure response, andsuicide attempts.
Client Assessment & Documentation: Conducts assessments on the date of client admission anddocuments patient progress and treatment recommendations in the EHR for provider review.
