Clinical Evaluator (FQHC)
Job Description
The Mandeville clinic is hiring for a Clinical Evaluator to assess patients for acuity of need, connect patients with resources and obtain patient medical history.
Minimum Requirements
● Masters degree in a social service field preferred or licensed nurse in good standing.
● Be over 18 years of age.
● Possess a valid Louisiana driver’s license
● Computer proficiency in Microsoft Excel (preferred)

Job Duties/Skills Required
● Maintains professional and ethical standards of conduct at all times.
● Promotes recovery, personal choice, and respect.
● Collect patient medical and social history including health screenings.
● Assess patient for acuity of need and scheduling within appropriate time frame.
● Capable of working with a multi-disciplinary team.
● Enters patient information into EHR in a timely manner with accurate information.
● Participate in staff meetings and complete trainings as required.
● Maintain and adhere to privacy standards including HIPAA.
● Screens telephone calls and visitors as directed; may refer calls to appropriate staff.
● Welcomes and is courteous to all persons entering the office.
● Coordinate with front office staff, providers, and crisis services to assist patient.
● Types or enters data requiring knowledge of special terminology.
● Proofreads, edits, and revises documents for accuracy, spelling, grammar, and format.
● May develop spreadsheets, databases, or other applications as directed.
● All other duties assigned by the Program Director.
