Job Description

Plans, organizes, and arranges outreach events for rapid testing in the community. Attends outreach events with team, conducting rapid testing and counseling, assisting patients in scheduling appointments with START CHC and elsewhere, and transporting patients to the clinic as needed. Become certified in counseling, testing, and referrals (CTR) of the rapid tests through LDH. Meet with patients as needed at the clinic to assist them in locating resources, making appts, etc. and coordinate referrals. Accompanies individuals to appointments through the use of company vehicle. Create and maintain tracking spreadsheets to assist in program effectiveness. Attention to detail in completing paperwork for CTR, including logs, documentation of testing, inventory of testing supplies, referrals, lab testing, etc. Work closely with Program Director on grant-related tasks. Assist in the clinic when requested and time permits.

Minimum Requirements

•High School Diploma or GED, Bachelor’s preferred. 

•Be over 18 years of age. 

•Possess a valid driver’s license.

•Exceptional interpersonal skills and ability to adapt to changing environments and audiences. 

•Previous CTR certification/experience a plus

Job Duties/Skills Required

•Coordinate, arrange, and organize community-based outreach events for the clinic.

•Assist with basic clinic functioning when not engaged in outreach-based events or directly assisting clients. 

•Will complete training to do testing and counseling of rapid testing for HIV, HCV, and Syphilis. Will perform this testing and counseling in clinic and community-based outreach events. 

•Transport client to clinic when indicated. Transport client to outside appointments or resources if needed. 

•Utilize evidenced based practices, including motivational interviewing and problem solving skills, when engaging individuals with their recovery.

•Provides frequent face-to-face contact in order to engage and maintain individual with clinic and any related/desired resources. 

•Maintains contact with service providers to encourage referrals and insure effective coordination of services. 

•Participates in supervision to review cases and issues related to level of service.

•Attend outreach and other community meetings to coordinate with other outreach workers and community service providers.

•Collaborate and coordinate services with appropriate community agencies.

•Develop and maintain excellent working relationships with community partners and work together to identify and improve processes that enhance service.

•Maintain electronic records, reports, and statistics necessary for program functioning and evaluation.

•Accept responsibility for professional competency as appropriate.

•Is familiar with and liaises with hospitals and community agencies.

•Advocate on behalf of clients and/or support them to advocate for themselves.

•Other duties as assigned.
