Job Description

Assess patient for acuity of need and scheduling within appropriate time frame, crisis intervention, collect patient medical and social history including health screenings, coordinate with front desk office staff, providers and crisis services to assist patient.

Minimum Requirements

● Masters degree in a social service field preferred OR licensed nurse in good standing.

● Be over 18 years of age.

● Possess a valid Louisiana driver’s license

● Computer proficiency in Microsoft Excel (preferred)

  • Demonstrates knowledge of agency and community resources and referral processes.

Job Duties/Skills Required

  • Capable of working with a multidisciplinary team. 

  • May develop spreadsheets, databases, or other applications as needed
